By booking and attending a Northern Skies Hypnotherapy session, either in person or online, all clients are acknowledging and agreeing to the following Statements, Terms & Conditions and Privacy Policy:
That clients are aware of the types of therapy they will be receiving, including their purpose, methods and possible effects and that they understand and consent in full to this. That they understand that the treatment may be brought to an end at any time and that success of the treatment depends on the client’s ability to enter into the hypnotic process and desire to create change in themselves. Clients also understand that as part of the sessions, they may be accessing painful memories and emotions with the intention of re-processing these in the therapeutic process, so some very short-term discomfort may be possible.
For hypnotherapy to be successful, it requires commitment and motivation on the side of the client. People react very differently to hypnosis, and it is their experience of what hypnosis, so results may vary from person to person.
While hypnosis and hypnotherapy are safe and natural by nature, if a client has a condition including, psychosis, schizophrenia or epilepsy, hypnotherapy may not be suitable for them and this must be stated by the client at the initial consultation phone call/email.
Any audio recordings or downloads from the Northern Skies Hypnotherapy Hypnosis site or received from this practice should never be used while operating heavy machinery, driving, or performing tasks which require concentration.
Specific conditions for online therapy:
Online therapy is not available to clients that are suffering from any diagnosed psychiatric condition, psychological illness or epilepsy nor anyone under the supervision of a psychiatrist.
It is required that clients provide the following information on the questionnaire:
• contact details for your next of kin and the correct physical address where you will be when you attend online therapy
• name and full practice address and telephone number of their GP and that they authorise the therapist to contact the GP if the therapist regards it as essential for either the wellbeing of the client or others.
It is also required that clients:
• will take part in all sessions free from the influence of either alcohol or drugs and will provide a list of any prescription medicines they are currently taking.
• will ensure that the environment in which sessions are undertaken shall be safe and free from distractions and will inform the therapist if there is anyone else present or monitoring the session.
• will not record the session (either by sound or visual means) without prior permission from the therapist.
• understand and accept that the therapist may terminate the session without warning if the therapist should determine that the session has been booked for some purpose other than to receive therapy, and that should this occur, the client will remain liable for any session fee that may have been paid in advance.
Deposit Payment – As there is a lot of preparation which goes into each session, we ask for a £45 deposit or full payment to confirm bookings. This is returnable at the last agreed session where only the balance will need be paid unless already paid in full, notwithstanding the cancellations, postponements, no shows policy below. PLEASE NOTE A BOOKING IS NOT CONFIRMED (and may be reallocated) UNTIL THE DEPOSIT HAS BEEN RECEIVED.
Cancellations, postponements, delays, no shows
Cancellations/postponements of appointments are sometimes unavoidable. However please give as much notice as possible. If a cancellation or postponement is within 48 hours, the 1-hour session will be reallocated to a mutually convenient time and date. In the event of late arrival, the chargeable duration of the session will be measured from the original agreed start time. No shows will forfeit the deposit.

Also information disclosed and matters discussed during any consultations or therapy.
Uses made of the personal information
Clients (those who have paid for services with us):
We use information held about you in the following ways:
· to carry out our obligations arising from any contracts entered into between you and us relating to hypnotherapy consultations and to provide you with the information and services that you request from us. In other words to provide you with therapy and help you achieve your goals
· to generally improve the quality of our strategies and services for yourself and other clients. On occasion this may entail that anonymous information taken extracted from the notes or sessions will be invigilated by a professional hypnotherapy supervisor
· to provide you with information or guidance we think will be helpful to you
Enquiries (those who have contacted us and have not paid for services):
We use information to respond to enquiries, answering any questions and offering suggestions for treatment and booking appointments.
Our lawful basis to collect, process and hold data is by consent of the data subject. By submitting your personal data or contacting us, you agree and provide lawful consent to using your data in this way.
Disclosure of your information
We will not share your confidential personal identifiable information with any third parties unless we are instructed or permitted to do so by you or we are required to do so by law or in special cases, such as a physical threat to you or others. In the event that we are legally compelled to disclose your Personally Identifiable or Personal Health Information to a third party, we will attempt to notify you unless doing so would violate the law or court order. From time to time some of the information taken in the notes or sessions will be invigilated by a professional hypnotherapy supervisor. This information will not include any personal or identifying information and remains anonymous.
Where and how we store your personal data
Any personal identifiable data that we collect from will be stored under password protected encryption, via the Microsoft OneDrive web storage application.
In addition certain anonymous (non identifiable) data relating to your case history and treatment strategy may be stored on Dropbox without encryption. This data cannot be traced to your identity without the use of a separate Client List Key which will be stored under password protected encryption, on Microsoft OneDrive. The purpose of this data is to provide easy access to your case notes for use in planning or during treatment sessions while fulfilling our agreed commitment to you. This data may also be used to develop strategies and improve our services for other clients.
Our lawful basis to collect, process and hold data is by consent of the data subject. By submitting your personal data, you agree to this transfer, storing or processing. We will take all steps reasonably necessary to ensure that your data is treated securely and in accordance with this privacy policy.
Unfortunately, the transmission of information via the internet and email is not completely secure. Although we will do our best to protect your personal data, we cannot guarantee the security of your data transmitted to us; any transmission is at your own risk. Once we have received your information, we will use strict procedures and security features to try to prevent unauthorised access.
Retention Period
Clients (those who have received paid for services with us):
All data may be stored indefinitely but will not be stored for a period of less than 8 years from the date of the client’s last visit or, if the client is a child, until his or her 25th birthday, or 26th birthday if the client was 17 when the treatment ended, for the following reasons:
· To provide continuity of service to clients who return for future treatments
· To fulfil legal obligations should case notes be required by a court of law
Enquiries (those who have contacted us and have not paid for services):
All personal identifiable and contact data to be stored for 1 year, allowing enquiry sufficient time to make a decision without pressure. If after the initial 12 months the enquiry does not become a client then the data will be erased within the next 12 months.
Your rights - access to information
The Act gives you the right to access information held about you. Your right of access can be exercised in accordance with the Act.
